Things to know prior to your consultation
How to book an appointment
We currently only accept bookings via phone. Please contact our reception staff at (03) 9500 1320 to book an appointment. Please remember you are required to bring a current referral letter in order to have your claim processed by Medicare. Telehealth consultations are only available for existing patients, and only on video platforms (phone calls are not eligible for a rebate).
What to bring on the day
Please bring your referral letter from your GP or specialist, your Medicare card, your Patient Authority/Consent Form, a list of medications you currently use, and recent X-rays and relevant imaging. The Patient Authority/Consent form, TAC forms and WorkCover patient forms can be found on our website under 'Forms'. Dr Harmer does her best to accommodate patients' individual circumstances, however children may be asked to stay in waiting room if disruptive.
Cancellation Policy
If you need to cancel your appointment please give at least 24 hours notice. Failing to give adequate notice will incur a charge equivalent to 50% of the consultation fee. If you need to reschedule an appointment, please try and provide a week's notice. This allows us to give your appointment to other patients who are waiting to be seen.
Confirmation
We do require prior confirmation from you to secure your appointment. If we do not receive confirmation from you for a given appointment, we cannot guarantee that you will be seen for that given appointment. Confirmation may be made via phone call, SMS message or email.
COVID-19
If you have ANY of these symptoms:
fever
cough
sore throat
shortness of breath
AND
​Travelled to (including transited through) a country or a state considered to pose a risk of coronavirus transmission in the 14 days before getting sick

Had close contact with someone diagnosed with or suspected of having coronavirus in the 14 days before getting sick, please do the following:
